  2. Fill the form and submit a copy to JAMB.

  3. Keep a copy for yourself for reference purposes, and

  4. Make a duplicate of it which you shall submit to the Institution you seek admission through Direct Entry.

Important Note: Ask whether the Institution you seek for admission sells Direct Entry form separately from JAMB. Some schools especially in the South East such as the Ebonyi State University (EBSU) have started accepting Direct Entry application directly. Therefore, if you bought DE form from JAMB office, and did not buy from the school, you may not be given admission. We advice that you buy DE form from school, if the school sells it instead from JAMB.
